Smooth Transition, Inc. has created a comprehensive
three phase program that meets the needs of our teens on various levels. Our first phase is designed to get those in the program
caught up in the basic life skills required for learning how to live on their own and in the world. We
offer participants a state of the art computer lab, classrooms and tutoring to ensure that they have the tools they need to
progress successfully through the program. All of our phases are accompanied by benchmarks, goals and objectives
and each successive phase enjoys chances for personal empowerment, responsibilities and as they transition to higher phases,
gives them the chance to celebrate their accomplishments and to share this success with others in the program.
Our linkage
in the local community represent strong working relationships and include a wide range of government, nonprofit, community
and local service organizations. Our partnership with these entities help us provide effective service
planning, a better coordination of program services, comprehensive and appropriate referrals, increased service quality and
efficiency. We hold regular events that draw upon the community at large and generate private support each
year, through building relationships with local corporations and businesses. Many of these relationships
helps provide employment opportunities and work experience to our at risk teens, thus furthering the goals and objectives
of the program.
We also offer a 10 week intensive program for students who need less one
on one mentoring and want to attain the skills they need faster. We offer this program as a credit program as well for
schools and programs who qualify. Statistically our students come away from our program with much higher levels of confidence
across the board. When completing our 10 week intensive program, students tested showed a 26% increase in overall career development
confidence levels and a 26% increase in career problem solving confidence, a 22% increase in career planning confidence,
a 22% increase in career goal setting confidence, a 27% increase in occupational information confidence and a 26% increase
in career self-assessment confidence levels. Program participants also saw an average 13.32% increase in overall perceived
self-worth and value upon completion of the program. In addition, program participants saw an average 37.5% decrease
in perceived achievement barriers. These barriers include age, physical, self-confidence, education and training,
financial, time, family concerns, fear of failure, fear of success, fear of the unknown, lack of focus and fear of making
the wrong decision (see graphs below).
